CLINIC POLICIES & PROCEDURES

A COVID-19 self-assessment questionnaire will be emailed to all patients 24 hours before their appointments. The questionnaire must be completed before their appointment, otherwise they may not be able to come for treatment. 

If a person tests positive during the self-assessment or pre-treatment follow up, they should complete the on-line COVID-19 questionnaire (https://covid-19.ontario.ca/self-assessment/) before contacting their family/walk-in doctor or Telehealth (Toll-free: 1-866-797-0000) for further instructions.

  • Upon arrival in the parking lot, either text or call me to confirm you have arrived. Please wait in your car until I contact you to come upstairs.

  • Unless you need assistance (eg. translator, parent/guardian of individual under 18/elderly), please come to your appointment alone.

  • You will need to wash your hands with soap and water before entering the clinic (washrooms are located just down the hallway from the clinic entrance). Wearing gloves does not substitute for hand hygiene.

  • All patients are required to wear either a mask or face covering while in the clinic (certain exemptions apply: https://www.ontario.ca/page/face-coverings-and-face-masks#section-2). If you do not have a mask, one will be provided for you.

  • Once your appointment is completed, you will need to leave the clinic as soon as possible so the room can be disinfected.
